Application for Termination of Service

Please read all instructions carefully before opening the online Application for Termination of Service.

1. Use this form only if you wish to stop receiving service from City Water, Light and Power. If you are moving to another location inside Springfield's service territory and wish to have your service transferred to your new address, use the Application for Transfer of Service to a New Location.
2. To ensure your will be able to successfully use this form, your computer should be equipped with Adobe Acrobat or Acrobat Reader 8.0 or higher. You might not be able to submit or print the form using an older version of Acrobat. For a free download of the most recent version of Acrobat Reader, click on this icon: This is the Get Acrobat Reader logo. Clicking on it will take you to the Adobe website, where you can download a free copy of the latest version of Acrobat Reader.
3. After opening the form, complete all appropriate fields. (Fields identified by an asterisk must be filled in before you will be allowed to print or submit the form. Other fields are optional.)
4. If you wish to print a copy of the form for your files or for manual submission (see #7 below for instructions on how to submit the form manually), click the Print Form button at the bottom of the form.
5. If you wish to clear the data you have input into all of the fields, click the Reset Form button at the bottom of the form.

8. CWLP will process your application for service termination within one business day of receipt. In some cases, we will be able to follow through on your request based solely on the information you provide in the application. However, in some cases, additional information or arrangements will be required, in which case a Customer Service Representative will contact you to work out details.
