REPORTING YOUR OWN METER READING

CWLP residential customers who wish to read your own meter(s) and report the readings to the CWLP Customer Service Office may use this form to do so. To electronically complete and submit the PDF version of this form, your computer must be equipped with Adobe Acrobat 5.0 (or a later version) or Adobe Acrobat Approvals 5.0. You can fill in the form online and print it out using Adobe Acrobat Reader, which you can download for free by clicking on the icon at the end of these instructions. However, you will not be able to submit the form electronically.

How To Use This Form

  1. Read your meter(s) following the instructions contained in How To Read Your Meter. (To open this document, you will need Adobe Acrobat Reader, which you can download for free by clicking on the icon at the end of this page.)
  2. Click on the "Open the Customer Meter Reading form" link at the end of this page and complete the form by clicking your cursor inside each box and typing. All fields preceded by an asterisk (*) must be completed in order for your meter reading to be processed.
  3. Submit your application in person or by mail, FAX or email:

    1. To submit it in person or by mail or FAX, print out your completed file and either bring or mail it to CWLP Customer Service Office, Room 101, Springfield, IL 62757; or FAX it to (217) 789-2026.
    2. To file it electronically, click on the Submit button at the end of the form. Your computer must be equipped with Adobe Acrobat or Adobe Acrobat Approvals to submit the form electronically.

Please be sure to return the completed form as soon as possible after you read the meter(s).
