City Water, Light & Power has prepared a number of publications for our customers who want to learn more about how to make your homes and businesses more energy and water efficient. These publications are available at no charge to anyone who receives service from or who owns property serviced by CWLP. For a description of our publications, click here. You can use the Efficiency Publications Order Form to place your order either in person, or by mail, FAX, or email.

To electronically complete and submit the PDF version of this form, your computer must be equipped with Adobe Acrobat 5.0 (or a later version) or Adobe Acrobat Approvals 5.0. You can open, fill in and print out the form using Adobe Acrobat Reader, which you can download for free by clicking on the icon at the end of these instructions. However, you will not be able to submit the form electronically.

How To Use This Form

  1. Access the form by clicking on the “Open Efficiency Publications Order Form” link below.
  2. Complete the form by placing your cursor inside each box and typing in the requested information or by clicking on a box to select an option. All sections or fields preceded by an asterisk (*) must be completed for your order to be processed.
  3. To submit your order in person or by mail or FAX, print out your completed file and either bring or mail it to the CWLP Energy Services Office, 2nd Floor, 231 S. 6th St., Springfield, IL 62701; or FAX it to (217) 789-2210.
  4. To submit your order electronically, click on the Submit button at the end of the form. Your computer must be equipped with Adobe Acrobat or Adobe Acrobat Approvals in order to submit the form electronically.
