City Water, Light & Power has prepared a number of publications for our customers who want to learn more about how to make your homes and businesses more energy and water efficient. These publications are available at no charge to anyone who receives service from or who owns property serviced by CWLP. For a description of our publications, click here. You can use the Efficiency Publications Order Form to place your order either in person, or by mail, FAX, or email.

To use the Word version of this form, your computer should be equipped with Microsoft Word. CWLP does NOT recommend attempting to complete and submit this form using any other word processing software.

How To Use This Form

  1. Access the form by clicking on the “Open Efficiency Publications Order Form” link below.
  2. Complete the form by placing your cursor inside each gray box and typing in the requested information or by clicking on a box to select an option. All sections or fields preceded by an asterisk (*) must be completed in order for your information to be processed.
  3. Save the completed application to your hard or floppy drive (please do NOT change the name of the file when you save it).
  4. To submit your order form in person or by mail or FAX, print out your saved file and either bring or mail it to the CWLP Energy Services Office, 2nd Floor, 231 S. 6th St., Springfield, IL 62701; or FAX it to (217) 789-2210.
  5. To submit your order electronically, click here to open an email message box and then attach your saved file.
