Online Services

At CWLP, we want to make it as convenient as possible for our customers to conduct business with us. One of the ways we do this is by providing you online access to the forms you need to order special utility services. The forms that are currently available online are described below. To access any of these forms, just click on the appropriate links. All forms are available as PDFs; some are also available as Word documents.

Although some of the PDF forms are available only in downloadable format, many can be filled out online and submitted to us electronically if your computer is equipped with Adobe Acrobat 5.0 (or a later version) or Adobe Acrobat Approvals 5.0. If you don't have either of these software packages, you can still open and fill out a PDF form online as long as your computer is equipped with Adobe Acrobat Reader, which you can download for free by clicking on the "Get Acrobat Reader" icon at the end of this page. However, you will not be able to successfully save or submit your form electronically using Adobe Acrobat Reader. You will need to print out the form and return it to CWLP by mail or FAX or in person.

Word forms can be filled out and submitted electronically if your computer is equipped with Microsoft Word. We DO NOT recommend that you attempt to electronically complete one of our Word forms using a different type of word processor. Doing so is likely to result in formatting errors that could prevent us from processing your request/application.

Application for New Service Although you must visit the Customer Service Office in person in order to sign up for new service, you can save some time by filling in the application form in advance and bringing it with you. This form is for persons who wish to obtain water or electric service from CWLP but who do not currently have such service.

Application for Transfer of Service to a New Location Current CWLP water or electric customers who are moving to a different location within CWLP’s service territory and who wish to have their service transferred from their current location to their new address may use this form.

PDF forms can be successfully submitted electronically only if you have a FULL version of Adobe Acrobat (not just Acrobat Reader.) Word forms submitted electronically may not be readable by CWLP if a wordprocessing application other than Microsoft Word is used by the customer.

Application for Termination of Service Current CWLP water or electric customers who will no longer need CWLP electric or water service in their name may use this form to request the termination of their service.

Application for Commercial Electric Service Persons who wish to request electric service from CWLP for a new commercial building or to request a service upgrade for an existing commercial facility may use this form.

Application for Temporary Service for a Festival or Other Special Event If you are planning to hold or participate in an outdoor festival or other special event for which electric service will be required, CWLP can provide temporary service according to your needs. You may use this form to order temporary service for this purpose. Please notify CWLP of your need for service at least two weeks prior to the event’s scheduled date.

CWLP Private Outdoor Lighting Application and Contract CWLP will install outdoor pole-mounted security lights for customers upon request, as long as the area to be lit has overhead electric service. Reporting Your Own Meter Reading CWLP residential customers who wish to read your own
meter(s) and report the readings to the CWLP Customer Service Office may use this form.

Energy and Water Efficiency Publications Order Form City Water, Light & Power has prepared a number of publications for our customers who want to learn more about how to make your homes and businesses more energy and water efficient. These publications, which are available at no charge to anyone who receives service from or who owns property serviced by CWLP, may be ordered by using this form.

Update Your Account Information In order to provide the best possible service, CWLP needs to maintain up-to-date contact information on all our customers. If any of the information, including phone numbers and contact person name, provided on your application for service has changed, please use this form to update your records.

Round Up To End Homelessness Donation Form CWLP customers can make regular contributions to agencies and services that aid the local homeless population by electing to round up their monthly bills to the nearest dollar. Automatic Withdrawal Authorization One of the easiest ways to make your bill payments is to participate in CWLP’s Direct Debit Payment program, which allows CWLP to make automatic withdrawals from your checking or savings account. You can use the Automatic Withdrawal Authorization form to sign up for Direct Debit Payment.

This form is available only in a downloadable format. When returned to CWLP, the form must be accompanied by a voided check or deposit slip from the checking or savings account you wish to pay your bills from.

Dealer Water Heater Rebate Application CWLP offers rebates to water heater vendors who both sell and install electric water heaters that qualify for our customer Water Heater Rebate. Heat Pump Rebate Application CWLP offers the Heat Pump Rebate XL, in the form of a bill credit, to customers who install qualifying heat pumps in their properties on or after November 1, 2007. Separate application forms are available for air-source heat pumps and for geothermal heat pumps. Units installed prior to November 1, 2007, might also qualify for a rebate under the utility's previous heat pump rebate program. A different application form should be used to apply for a rebate under the old heat pump rebate program.

When returned to CWLP, your rebate application must be accompanied by proof of purchase.

Dealer Heat Pump Rebate Application To encourage HVAC dealers to promote and sell heat pumps within the CWLP service territory, CWLP offers a rebate to dealers that is equal to 50% of the customer's rebate or $500, whichever is less. Insulation Rebate Application CWLP offers a rebate, in the form of a bill credit, to customers who enhance the amount of insulation in existing properties or who install above-code levels of insulation in new construction.
