APPLICATION FOR TRANSFER OF SERVICE
TO A NEW LOCATION

This form should be used to request termination of service at your current address ONLY if you are also requesting reinstatement of service under your name at a different address. To terminate service without reinstatement at a different location, use the Application for Termination of Service form.

Current CWLP water or electric customers who are moving to a different location within CWLP’s service territory and who wish to have their service transferred from their current location to their new address may use the Application for Transfer of Service to a New Location to do so. To fill in and submit the PDF version of this form electronically, your computer must be equipped with Adobe Acrobat 5.0 (or a later version) or Adobe Acrobat Approvals 5.0. If you do not have either of these programs, you can open, fill in and print the PDF form using Adobe Acrobat Reader, which you can download for free by clicking on the icon at the end of these instructions. However, you will not be able to submit the form electronically.

How To Use This Form

  1. Access the form by clicking on the “Open Application for Transfer of Service” link below.
  2. Answer all applicable questions. Just place your cursor inside each box and begin typing. All fields preceded by an asterisk (*) must be completed in order for your form to be processed.
  3. Submit your application in person or by mail, FAX or email:

    1. To submit it in person or by mail or FAX, print out your saved file and either bring or mail it to the CWLP Customer Service Office, Room 101, Municipal Center West, Springfield, IL 62757; or FAX it to (217) 789-2026.
    2. To file it electronically, click on the Submit button at the end of the form. You must have Adobe Acrobat or Adobe Acrobat Approvals installed on your computer in order to submit this form electronically.

CWLP will process your application for service transfer within one business day of receipt. In some cases, we will be able to follow through on your request based solely on the information you provide here. However, in some cases, additional information or arrangements will be required, in which case a Customer Service Representative will contact you to work out details.
