This form should be used to request termination of service at your current address ONLY if you are also requesting reinstatement of service under your name at a different address. To terminate service without reinstatement at a different location, use the Application for Termination of Service form.
Current CWLP water or electric customers who are moving to a different location within CWLP’s service territory and who wish to have their service transferred from their current location to their new address may use the Application for Transfer of Service to a New Location to do so. To fill in and submit the PDF version of this form electronically, your computer must be equipped with Adobe Acrobat 5.0 (or a later version) or Adobe Acrobat Approvals 5.0. If you do not have either of these programs, you can open, fill in and print the PDF form using Adobe Acrobat Reader, which you can download for free by clicking on the icon at the end of these instructions. However, you will not be able to submit the form electronically.
How To Use This Form
CWLP will process your application for service transfer within one business day of receipt. In some cases, we will be able to follow through on your request based solely on the information you provide here. However, in some cases, additional information or arrangements will be required, in which case a Customer Service Representative will contact you to work out details.
