This form should be used to request termination of service at your current address ONLY if you are also requesting reinstatement of service under your name at a different address. To terminate service without reinstatement at a different location, use the Application for Termination of Service.
Current CWLP water or electric customers who are moving to a different location within CWLP’s service territory and who wish to have their service transferred from their current location to their new address may use the Application for Transfer of Service to a New Location to do so. To use the Word version of this form, your computer should be equipped with Microsoft Word. CWLP does NOT recommend attempting to complete and submit this form online using any other word processing software.
How To Use This Form
CWLP will process your application for service transfer within one business day of receipt. In some cases, we will be able to follow through on your request based solely on the information you provide here. However, in some cases, additional information or arrangements will be required, in which case a Customer Service Representative will contact you to work out details.
