APPLICATION FOR TERMINATION OF SERVICE
 (Word version)

This form should be used to request termination of service at your current address ONLY if you are NOT also requesting reinstatement of service under your name at a different address. To transfer service from your current address to a different location, use the Application for Transfer of Service to a New Location.

Current CWLP water or electric customers who wish to cease receiving CWLP electric or water service in their names may use the Application for Termination of Service to request service termination. To use the Word version of this form, your computer should be equipped with Microsoft Word. CWLP does NOT recommend you attempt to complete and submit this form online using any other word processing software.

How To Use This Form

  1. Access the form by clicking on the “Open Application for Termination of Service” link below.
  2. Answer all applicable questions. Just place your cursor inside each gray box and begin typing. All fields preceded by an asterisk (*) must be completed in order for your form to be processed.
  3. Save the completed application to your hard drive (please do NOT change the name of the file when you save it).
  4. Submit your application in person or by mail, FAX or email:

    1. To submit it in person or by mail or FAX, print out your saved file and either bring or mail it to the CWLP Customer Service Office, Room 101, Municipal Center West, Springfield, IL 62757; or FAX it to (217) 789-2026.
    2. To file it electronically, click here to open an email message box and then attach your saved file. You should have Microsoft Word to do this.

CWLP will process your application for service termination within one business day of receipt. In some cases, we will be able to follow through on your request based solely on the information you provide here. However, in some cases, additional information or arrangements will be required, in which case a Customer Service Representative will contact you to work out details.
