APPLICATION FOR TEMPORARY SERVICE
FOR A FESTIVAL OR OTHER SPECIAL EVENT

This form should be used only to request temporary electric service for a festival or other short-term special event and NOT for service at a permanent location or construction site.

Persons who wish to request temporary electric service from CWLP for a festival or other short-term special event may use the Application for Temporary Service for a Festival or Other Special Event to do so. The purpose of this form is to establish initial contact with the CWLP T&D Engineering Department and to provide us as much information about your temporary service needs as you can. If there are any questions you cannot answer at this time, leave those spaces blank. We will fill them in after a CWLP representative has a chance to talk to you.

To complete and submit the PDF version online, your computer must be equipped with  Adobe Acrobat Standard 5.0 (or a later version), Adobe Acrobat Professional 5.0 (or a later version), or Adobe Acrobat Approvals 5.0. If you have access only to Adobe Acrobat Reader, you will be able to open, fill in and print out the PDF version, but you will not be able to submit the form electronically. How To Use This Form

  1. Access the application by clicking on the “Open Application for Temporary Service” link below.
  2. Answer all applicable questions. Just place your cursor inside each box and begin typing. All fields preceded by an asterisk (*) must be completed in order for your form to be processed.
  3. Submit your application in person or by mail, FAX or email:

    1. To submit it in person or by mail or FAX, print out your completed file and either bring or mail it to Mike Workman, CWLP Electric Engineering Center, 1008 E. Miller Street, Springfield, IL 62702; or FAX it to (217) 789-2082.
    2. To file it electronically, click on the Submit button at the bottom of the form. You must have Adobe Acrobat or Adobe Acrobat Approvals installed on your computer in order to submit this form electronically.

Once your application has been reviewed by the CWLP T&D Engineering Office, the Customer Contact person listed on the application will be contacted by a CWLP representative to work out specific details of the request.
