Instructions for
CWLP PRIVATE OUTDOOR LIGHTING
APPLICATION AND CONTRACT

This application does not constitute an enforceable contract until approved and signed by both parties.

Persons who wish to request a pole-mounted outdoor security light from CWLP may use the Private Outdoor Lighting Application and Contract to do so. The purpose of this form is to establish initial contact with the CWLP T&D Engineering Department and to provide us as much information about your security lighting needs as you can. If there are any questions you cannot answer at this time, leave those spaces blank. We will fill them in after a CWLP representative has a chance to talk to you.

CWLP will provide outdoor, pole-mounted security lighting for a monthly fee to qualifying electric customers as facilities are available (see Program Notes and Restrictions below for limitations to this offer). To use the Word version of the Private Outdoor Lighting Application and Contract, your computer should be equipped with Microsoft Word. CWLP does NOT recommend attempting to complete and submit this form online using any other word processing software.

How To Use This Form

  1. Access the form by clicking on the "Open Outdoor Lighting Application" link below.
  2. Answer all applicable questions. Just click cursor inside each gray box and begin typing or select one of the available options. All fields preceded by an asterisk (*) must be completed in order for your form to be processed.
  3. Save the completed application to your hard drive (please do NOT change the name of the file when you save it).
  4. Submit your application in person or by mail, FAX or email:

    1. To submit it in person or by mail or FAX, print out your saved file and either bring or mail it to Mike Workman, CWLP Electric Engineering Center, 1008 E. Miller Street, Springfield, IL 62702; or FAX it to (217) 789-2082.
    2. To file it electronically, click here to open an email message box and then attach your saved file. Your computer should be equipped with Microsoft Word in order to submit the form electronically.

Upon receipt and review of your application, a CWLP representative will contact you to finalize your request, establish an installation date and location, and sign the contract.

Program Notes and Restrictions:

  1. CWLP will have the final decision regarding the location of the security light(s).
  2. All installed facilities will remain the property of CWLP.
  3. Security lighting is not available in most areas where electric service is located underground. CWLP also reserves the right to deny security lighting in other areas as deemed necessary to prevent damage or other problems to utility, other public or private property.
  4. Lights will be installed only on a CWLP utility pole and only if the pole can accommodate the light and if the light will not interfere with CWLP's primary use of the pole.
  5. Lights will not be installed in an area where overhead 120-volt service is not immediately available, unless the customer pays the cost of installing facilities necessary to provide 120-volt service to the light.
  6. No more than three lights (installed either separately or together) will be allowed for any commercial or industrial customer, unless those lights are installed on public property, such as street or alley right-of-way, or installed on existing poles. Security lighting will not be provided for the purpose of overall lighting of parking lots, private streets or roadways.
  7. CWLP has the right to deny security lighting service to rental properties, unless the property owner agrees to sign the contract and be responsible for the cost of service.
  8. CWLP reserves the right to cancel any contract if excessive maintenance or replacement costs result from vandalism or other causes.
