APPLICATION FOR COMMERCIAL ELECTRIC SERVICE

Persons who wish to request electric service from CWLP for a new commercial building or to request a service upgrade for an existing commercial facility may use the Application for Commercial Electric Service to do so. The purpose of this form is to establish initial contact with the CWLP T&D Engineering Department and to provide us as much information as you can about your service needs. If there are any questions you cannot answer at this time, leave those spaces blank. We will fill them in after a CWLP representative has a chance to talk to you.

To complete and submit the PDF version online, your computer must be equipped with  Adobe Acrobat Standard 5.0 (or a later version), Adobe Acrobat Professional 5.0 (or a later version), or Adobe Acrobat Approvals 5.0. If you have access only to Adobe Acrobat Reader, you will be able to open, fill in and print out the PDF version, but you will not be able to submit the form electronically. How To Use This Form

  1. Access the form by clicking on the "Open the Application for Commercial Electric Service" link below.
  2. Answer all applicable questions. Just click cursor inside each box and begin typing or click on a "check" box to select an option. All fields preceded by an asterisk (*) must be completed in order for your form to be processed.
  3. Submit your application in person or by mail, FAX or email:

    1. To submit it in person or by mail or FAX, print out your completed file and either bring or mail it to Mike Workman, CWLP Electric Engineering Center, 1008 E. Miller Street, Springfield, IL 62702; or FAX it to (217) 789-2082.
    2. To file it electronically, click on the Submit button at the end of the form. You must have Adobe Acrobat or Adobe Acrobat Approvals installed on your computer in order to submit this form electronically.

Once the CWLP T&D Engineering Office has reviewed your application, a CWLP representative will contact the Customer Contact person listed on the application to work out specific details of the request.
