Instructions for
APPLICATION FOR COMMERCIAL ELECTRIC SERVICE
 (Word version)

Persons who wish to request electric service from CWLP for a new commercial building or to request a service upgrade for an existing commercial facility may use the Application for Commercial Electric Service to do so. The purpose of this form is to establish initial contact with the CWLP T&D Engineering Department and to provide us as much information as you can about your service needs. If there are any questions you cannot answer at this time, leave those spaces blank. We will fill them in after a CWLP representative has a chance to talk to you.

To use the Word version of this form, your computer should be equipped with Microsoft Word. CWLP does NOT recommend attempting to complete and submit this form using any other type of word processing software.

How To Use This Form

  1. Access the form by clicking on the "Open the Application for Commercial Electric Service" link below.
  2. Answer all applicable questions. Just click cursor inside each gray box and begin typing. All fields preceded by an asterisk (*) must be completed in order for your form to undergo preliminary processing.
  3. Save the completed application to your hard drive (please do NOT change the name of the file when you save it).
  4. Submit your application in person or by mail, FAX or email:

    1. To submit it in person or by mail or FAX, print out your saved file and either bring or mail it to Mike Workman, CWLP Electric Engineering Center, 1008 E. Miller Street, Springfield, IL 62702; or FAX it to (217) 789-2082.
    2. To file it electronically, click here to open an email message box and then attach your saved file. Your computer should be equipped with Microsoft Word in order to submit the form electronically.

Once the CWLP T&D Engineering Office has reviewed your application, a CWLP representative will contact the Customer Contact person listed on the application to work out specific details of the request.
