Account Information Update – Commercial (Word version)

Please read all instructions carefully before opening the Word version of the online Commercial Account Information Update form.

1. Use this form to update contact information in your CWLP commercial account.
2. To successfully submit this form electronically, your computer should be equipped with Microsoft Word. If you have some other type of word processor, please do not attempt to make an electronic submittal, as your form might be unreadable by CWLP.

NOTE: If you have a word processor other than Microsoft Word, you can still type your information into this form, then print it and submit it manually (see #5 below for instructions on how to submit the form manually).  

3. After opening the form, complete all appropriate fields. (Fields identified by an asterisk must be filled in.)
4. To submit this form electronically, save the completed form to your hard drive (as a Word document only). Do NOT change the file name when saving. After saving the form, close Word. (If you have an older version of Word, you might need to click the BACK option in the main menu bar. A dialogue box will appear asking if you want  to save the changes that have been made to the document. Click NO.) This will return you to this instruction page. Once back at this page, click here to open a pre-addressed email message box, attach your saved file to the email, then click Send.
