CUSTOMER INFORMATION UPDATE
(Word version)

Whenever the contact information you provided CWLP on your application for service changes, we need to know about it so we can update your account records. This helps us ensure the best, most reliable service possible to you and all our customers. To learn more about how having updated account information can help CWLP provide better customer service, click here.

To update your telephone numbers or other account information, you can use the Customer Information Update form.

To use the Word version of this form, your computer should be equipped with Microsoft Word. CWLP does NOT recommend attempting to complete and submit this form using any other word processing software.

How To Use This Form

  1. Access the form by clicking on the “Open Customer Information Update Form” link below.
  2. Complete the form by placing your cursor inside each gray box and typing in the requested information or by clicking on a box to select an option. All sections or fields preceded by an asterisk (*) must be completed in order for your information to be processed.
  3. Save the completed application to your hard or floppy drive (please do NOT change the name of the file when you save it).
  4. To submit your information update in person or by mail or FAX, print out your saved file and either bring or mail it to the CWLP Customer Service Office, Room 101, Municipal Center West, Springfield, IL 62757; or FAX it to (217) 789-2026.
  5. To submit your update electronically, click here to open an email message box and then attach your saved file. You should have Microsoft Word to do this.
