To enable you to participate in the Round Up, we provide three online versions of the Round Up To End Homelessness donation form for your convenience:
     PDF format – Allows for electronic form submission by
     customers whose computers are equipped with a FULL
      version of Adobe Acrobat 5.0 or higher. This PDF form
      CANNOT currently be successfully submitted using only
      Acrobat Reader.
            To use this form, (1) click on the box indicating the
      type of monthly donation you wish to make (if you elect
      to have a set amount added to your bill each month, be
      sure to fill in the dollar amount); (2) complete the remainder
      remainder of the form; then (3) click the submit button.

      Word format – Allows for electronic form submission by
      customers whose computers are equipped with Microsoft
      Word. We DO NOT recommend attempting to submit this
      form using a word processing application other than Word,
      due to the unpredictability of the quality of the document
      on the receiving end.
            To use this form, (1) click on the box indicating the
      type of monthly donation you wish to make (if you elect
      to have a set amount added to your bill each month, be
      sure to fill in the dollar amount); (2) complete the remainder
      of the form; (3) save the form to your hard or floppy drive
      (do NOT change the name of the form when you save it);
      then, (4) to submit your saved file, click here to open a
      a pre-addressed email message box and then insert
      (attach) your saved file and send the email.

      Downloadable format – For use by customers who are
      unable or prefer not to submit their donation forms
      electronically. This form can be printed out, completed
      manually, and mailed (to Round Up, CWLP Customer
      Service Office, Municipal Center West, Springfield, IL
      62757) or faxed (to 789-2026).

If, in the future, you decide to cancel your donation, you can do so by simply contacting the CWLP Customer Service Office at 789-2030.
